The Tuberous Sclerosis Alliance (TS Alliance) invites health care professionals to add your name to our referral list.  Board-certified physicians and other health care professionals who evaluate and treat individuals with tuberous sclerosis complex (TSC) are encouraged to join.

The list is not for public dissemination but will be used by the TS Alliance staff to assist constituents who request a referral to a health care professional in their area.  Please note that being on the list does not in any way suggest the TS Alliance’s endorsement or preference for any particular individual or group.  

To be included on the healthcare professionals’ referral list, please complete the Health Care Professional Referral Information Form and return it to the TS Alliance by:
